Welcome to Reform Athletics
Every plan here is built from scratch, so the more you share, the better it fits. This takes about 15 minutes. Everything stays between you and your coach.

Policy settings
This Client Service Agreement ("Agreement") is between Reform Athletics LLC ("Reform Athletics," "Coach") and the client ("Client"), effective the start date below.
1. Services
Reform Athletics will provide the selected service as described in the plan summary above. All programming is custom-built for Client; no templates are used.
2. Investment and payment
Payments are processed securely through Stripe. Services begin once payment is received.
3. Scheduling, cancellations and late arrivals
Sessions must be cancelled or rescheduled at least 24 hours in advance. Sessions cancelled with less notice, or missed without notice, are counted as completed and are not refunded or credited. If Client arrives late, the session still ends at its scheduled time; after 15 minutes without contact, the session is treated as a no-show. If Coach must cancel, the session will be rescheduled at no cost.
Packages. Package sessions must be used within 45 days of the first session. Unused sessions expire and are non-refundable and non-transferable unless Coach agrees in writing.
Monthly plans. Monthly plans renew automatically each month on the billing date. Client may cancel at any time with at least 7 days' notice before the next billing date; service continues through the end of the paid month. Partial months are not refunded.
4. Nutrition guidance
Nutrition guidance from Reform Athletics is general wellness coaching. It is not medical nutrition therapy and is not a substitute for advice from a physician or registered dietitian. Client is responsible for disclosing all food allergies, intolerances and medical conditions, and for consulting a physician before making significant dietary or supplement changes.
5. Client responsibilities
Client agrees to provide complete and accurate health information, to update Coach promptly about any change in health, injury, medication or pregnancy status, and to stop exercising and tell Coach immediately if they feel pain, dizziness, shortness of breath or discomfort. For in-home sessions, Client agrees to provide a safe, clear training space, to secure pets during sessions, and confirms that any equipment Client owns is in safe working condition.
6. Results
Results depend on many factors, including consistency, nutrition, sleep, stress, genetics and health history. Reform Athletics does not guarantee any specific result, including weight loss or body composition change.
7. Privacy
Reform Athletics keeps Client's personal and health information confidential and uses it only to design and deliver Client's program. It will not be sold or shared without Client's permission, except where required by law or in an emergency.
8. Ending this agreement
Either party may end this Agreement with written notice (email or text). Payments for sessions already delivered, expired package sessions, and the current paid month are non-refundable. Coach may end services immediately if Client's health or conduct makes training unsafe, in which case unused prepaid package sessions will be refunded on a pro-rated basis.
Founding client rate. As a founding client, Client's rate for this service is locked at for as long as Client's membership or package remains continuous. If Client's membership or package ends, Client may restart at this founding rate within 30 days of the end date. After 30 days, Reform Athletics' then-current rates apply.
9. General
This Agreement, together with the Liability Waiver and Release, is the entire agreement between the parties and may be changed only in writing. It is governed by the laws of the State of Tennessee. If any part is found unenforceable, the rest remains in effect. Both parties agree that electronic signatures on this Agreement are legally binding.

Liability Waiver and Release
Please read carefully. This document affects your legal rights.
I, the participant, want to take part in fitness training, coaching, nutrition guidance and related activities ("Activities") provided by Reform Athletics LLC, its owner Priscilla Claude, and its trainers, contractors and agents (together, "Reform Athletics"). Activities may take place in a gym, in my home, outdoors, at an assisted-living or group facility, or online.
1. Risks I understand and accept
I understand that exercise involves inherent risks, including but not limited to muscle strains and sprains, joint, bone and back injuries, falls, injuries from equipment, heat illness, fainting, abnormal blood pressure or heart rhythm, heart attack, stroke, and in rare cases death. I understand that these risks exist even when Activities are properly supervised. I knowingly and voluntarily accept all of these risks.
2. My health
I confirm that the health information I provided is complete and true. I have been advised to get clearance from my physician before beginning any exercise or nutrition program, and if I answered yes to any health screening question, I agree to obtain that clearance. I will tell my coach right away about any change in my health, and I will stop and speak up if I feel pain, dizziness, shortness of breath or any unusual symptom.
3. Release of liability
To the fullest extent allowed by Tennessee law, I release and agree not to sue Reform Athletics for any injury, illness, loss or damage to me or my property that arises from my participation in Activities, including claims arising from the ordinary negligence of Reform Athletics. This release does not apply to gross negligence or intentional misconduct. I agree to indemnify Reform Athletics for claims brought by others that arise from my own actions. This release binds me, my heirs, my estate and anyone acting on my behalf.
4. Training location and equipment
For sessions in my home or another location I choose, I am responsible for providing a safe space and for the condition of any equipment I own. Reform Athletics is not responsible for damage to my property that occurs during normal training activity.
5. Nutrition and supplements
I understand that nutrition and supplement guidance is general wellness coaching, not medical advice, diagnosis or treatment. I will consult my physician before changing my diet or taking supplements if I have a medical condition, take medication, or am pregnant or breastfeeding.
6. Emergencies
If I need emergency care, I authorize Reform Athletics to call emergency services and to contact my emergency contact, listed on my intake. I am responsible for any resulting medical costs.
7. Photos and video
Reform Athletics will never share my progress photos, measurements or likeness without my choice below.
8. Agreement
I have read this waiver, I understand it, and I am signing it voluntarily. I agree that my electronic signature is legally binding. This waiver is governed by the laws of the State of Tennessee and applies to all Activities, now and in the future, unless I revoke it in writing.
